  • Plot: 2 years after the events of the first film, Alice Miller is tracking SCP-096, an S.C.P creature who escaped the facility alongside many other dangerous entities. Alice is knocked unconscious by the creature and loses him. She is found by her sister, Carol, and she asks for her help; she agrees. Willard Kane witnesses the creature, and Commissioner Paul Earle orders him to kill it, but he refuses as he has made it clear that Alice vowed to capture the creatures if possible.
  • Earle orders the hunt for SCP-096, while Alice and Carol track down the creature. He proves more powerful than the Plague Doctor and overpowers them completely. Willard defies his orders and abandons the L.A.P.D, but Earle follows and attacks him. Willard shoots him through the chest and leaves him to be killed by the creature.
  • Alice and Carol reunite with Willard, who joins them on their hunt. The creature brutalizes Hal McKenna, but Deckard Walker, the identical twin brother of Dave Walker, saves McKenna and takes him to the hospital. McKenna explains to Alice that if the creature is unable to be recaptured, she must kill it.
  • Charles Owens, the corrupt police captain of the L.A.P.D, attempts to kill SCP-096, but it escapes and Alice shoots Owens. He refuses to dismiss the order to kill the creature, and Willard shoots him in the head. Willard persuades police chief John Tompkins to dismiss the order, but the creature kills Tompkins and injures Willard. Alice, Carol, Gabriel Santoro, and Walker chase the creature while Willard affirms the office of commissioner and orders the hunt for SCP-096 to only be occupied by Alice and the group.
  • Several cops defy the order and follow the group, but Alice shoots at them and threatens to kill them if they defy Willard's orders again. SCP-096 severely injures Carol, while Santoro and Walker are knocked unconscious. Acknowledging that the creature cannot be recaptured, Alice shoots it through its torso 16 times, fatally. Carol is taken to the hospital, while Alice explains to Walker about what happened to his brother. She apologizes to him, but he tells her not to blame herself for what happened. The creature, still fatally injured but not dead, ambushes the hospital and kills multiple doctors and patients except for McKenna, whom Alice tells to run.
  • Alice chases SCP-096 down in an ally where it becomes too exhausted to sprint. Alice shoots the creature through its head and finishes it off. Carol recovers, and Willard is officially titled the commissioner. Alice and Carol bid each other farewell before Alice herself roams the city to hunt more creatures as the end credits roll.
  • In a post-credits scene, Alice leaves the courtroom to prepare for the next trial in her battle for custody over her daughter Alexis against her ex-husband Bill Mason.
