  • Scientist Alice Miller, who has studied SCP-049 for two years, volunteers to hunt down the dangerous creature, but Michael Quinto, the manager of the facility, refuses to retrieve him. Alice disobeys the order and takes matters into her own hands, while her older sister, Carol, decides to go with her. SCP-049 slaughters a family of three with a potent poison in his syringe; he is dubbed the "Plague Doctor" as a result. Alice and Carol are outrun by the creature after finding him, before Quinto and the guards of the facility arrive to arrest them. They kill the guards but spare Quinto, who reluctantly joins them on the hunt for the creature.
  • Willard Kane, a police captain, is sent to hunt the creature down and kill him with the help of his squad members Jacob Graham, Deckard Owens, and Dave Walker. Alice, Carol, and Quinto create a cloaking device, as well as high-profile robotic mech suits to protect other people from SCP-049. The creature is caught murdering another family by the police, but he escapes and leaves Kane humiliated and enraged. Alice decides that the creature must be killed.
  • The creature kills Walker and brutalizes Owens and Graham, before disappearing into smoke. Alice, Carol, and Quinto team up with Kane, who saves them from SCP-049. Quinto betrays them, revealing that he released the creature and intends to let him kill them. Alice shoots him dead before the group escapes to pursue the creature. They don the mech suits and fight him, but he overpowers them and escapes once again.
  • Willard sets out to inform the entire police force that the creature is too strong for only a few fighters. Alice and Carol engage the creature in a final fight. While Carol is severely injured, Alice is knocked unconscious before the police arrive and save them. Willard commands them to hold their fire, but SCP-049 sprints towards them, and they shoot him dead.
  • In the aftermath, Alice and Carol recover in the hospital, while Willard incinerates the creature in a furnace. Alice deduces that other SCP creatures have escaped and vows to either kill or recapture them as the end credits roll.
