  • Plot: Marcus Black and Dakota are now married, and are expecting a child; Dakota's brother Gabriel has quit his job to become their next-door neighbor; and Larry Kress is the new police commissioner. The incarcerated Virginia Walker, who was the previous commissioner, manages to escape her imprisonment. Having anticipated this, Black and his wolf, Salazar, head out of the cabin and into the wild, but not before asking Gabriel to keep Dakota safe. With the snowstorm approaching, Black once again uses it to his advantage.
  • Mayor Gardner announces that Walker is on the loose, but secretly knows that she is in the forest. Gardner, having become friends with Black, tells the latter that he hopes he can kill her. Mark Quinn, having also quit his job, joins Black on the hunt. Realizing that Walker is not alone, they fight her convict allies from the prison she was sent to. Walker discovers the lifeless, deteriorating body of Cohan Gaines, and, in retaliation finds Black and seriously injures him.
  • Before Quinn takes Black to Dakota, she reveals that she killed Mayor Gardner after he snitched her to Black. Quinn shoots her arm and escapes with the injured Black. Dakota manages to nurse him back to health, giving him at least 2 days to fully recover. Gabriel, although knowing he cannot kill Walker, decides to go after her to give Black more time to heal. Gabriel slices Walker's leg open, just before she stabs him to death.
  • Black, having recovered, realizes that Gabriel is dead. He goes after Walker, vengeful for Gabriel's death, and sets bear traps for her group. Most of the group get caught in the traps, and Black brutally kills them, finally battling Walker; before he can kill her, he is attacked by a martial arts member of her convict group.
  • Dakota goes into labor, with Quinn being the only help she has, but calls her mother, Marion, over to assist him. Larry and a squad of cops arrive to help Black; all of them but Black and Kress are killed. Dakota has complications, but survives and manages to give birth to a daughter, with Quinn complimenting that Gabriel would've loved to see her. The martial artist, revealed to be Deckard Gaines, the older brother of Cohan, finds Black and knocks Kress unconscious, but not before telling Walker to run. After a bloody final fight, Black brutally rams his hand through Gaines' chest and rips his heart out.
  • In the aftermath, Black wakes up Kress, and they both conclude that Walker is still at large. They decide that they can deal with her next time, and head to the cabin. Black meets his newborn daughter, and he asks Dakota what to call their child; she replies "Chloe". Walker discovers Deckard's corpse, and vows revenge against Black as the end credits roll.
