In the gritty, crime-ridden streets of a decaying city, a former CIA operative named Al Simmons meets a tragic end. Betrayed and killed by his own employer, he strikes a deal with Malebolgia, the ruler of Hell, in a desperate bid to see his wife Wanda again. Malebolgia transforms him into Spawn, a demonic anti-hero with incredible powers.
