After Mr. Freeze went to Arkham Asylum willingly, Bruce Wayne learns of a horrific secret that nobody knew about for the longest time. Dexter Dent, Gotham's last District Attorney whom everyone believed to have been killed in the nuclear explosion in the city followed by a flood that granted Bruce a ticket to living in peace, was still alive. Description of Alfred Pennyworth (Flashback/Hallucination):

During the final battle in the city of Bludhaven, specifically in the Justice Building where Mayor Garfield Lynns (who is later revealed to be the Firefly) normally is, Bruce is led down a hallway by the Joker, who lets him trip over a piece of wire that releases the Scarecrow's fear gas inside the room of mirrors that Batman is now trapped in. He sees many of his dead loved ones, and adversaries, before eventually fighting back and tracking down the Joker inside the Bludhaven Justice Building, where they have their throw of hands.
