Francisco García Escalero is a lonely and marginalized man who lives in a shanty town in Madrid, near the Almudena cemetery. Since childhood, he has felt a morbid attraction to death and corpses, and suffers from schizophrenia, alcoholism and suicidal tendencies. After leaving prison, where he served a sentence for raping a young woman, he begins to hear voices ordering him to kill and desecrate beggars and prostitutes. Between 1987 and 1993, he murdered eleven people, whom he mutilated, burned and devoured. The police are unable to find him, until one day he turns himself in after killing a colleague in the psychiatric hospital. The film tells his chilling story, from his childhood to his confession, and shows his disturbed mind and his macabre rituals.
